SeniorWise Care Management Welcomes New Geriatric Care Manager

Marlton, NJ – Christopher Fritzsche, MSW, LSW of Pine Hill, has joined the geriatric care management staff of  SeniorWise Care Management, a division of Samaritan Healthcare & Hospice, announced SeniorWise Director Nancy Carman, MA, CMC.

 

As a geriatric care manager, Fritzsche meet with clients where they live and conducts a comprehensive needs assessment of their health, mental and nutritional status and safety risks within their home, among other concerns. He creates a care plan to share with the client and family, helps arrange and monitor in-home services or assists with moving a loved one, if needed, to or from a retirement community, assisted living residence or nursing care facility.

Fritzsche has spent most of his career as a case manager, working with the elderly and veterans. Most recently, Fritzsche worked with the Camden County Global Options Medicaid Waiver Program implementing resources to assist elders and the disabled population to remain independent in the community.
